Actually it's just language, title and identifier. The identifier is always created, so you don't need to worry about that.

Of course, always including the author is a good idea, it's just not required by the standard.

ERROR: 9781612100036.epub/OEBPS/content.opf(14): unfinished element

I believe this error was actually caused by there only being one item in the table of contents and so it was not being generated correctly? When I added in About the Author to the table of contents (this story was short and had no chapters) so that it had two items that error did not come up on the validation check.

Either that or the updated version of Calibre did the conversion to epub differently.

Have you tried to remove that tag entirely?

Use FlightCrew for initial epub checking. It has much better error reports and if it gives no errors chances are it validates in ePubCheck as well.
