Converting from epub3 to epub2 has restored the italics. I am extremely grateful as the thought of manually editing the css of about 90 books filled me with dread.

What is class epigraph in CSS?

Another thing to try. Download the eBooks from Kobo as ePub and use calibre to send them to your Kobo as KePub and see what happens. The code as is now looks very convoluted.

Have you tried viewing this eBook as KePub in the calibre viewer? Maybe the conversion to KePub caused the problem. You aren't seeing it correctly in the calibre viewer because it's being viewed as ePub.

Sirtel's suggestion to convert from ePub3 to ePub2 solved the problem completely!

Jon, please go back and review the differences between an ePub and a kepub when not using a Kobo ereader. The spans added are do nothings otherwise.

I think that now you have passed the 10 post mark, moderation should no longer be an issue.

Looking at the results? Like font-style: italic, font-align: center and with a top margin.
