Error in converting files html --> Mobipocket

I have a file that has a set of end notes in it. I was trying to link the endnotes to the number in the text, and then output the file in both the epub format and the mobi format. Using the ebook-convert command line, the epub converted fine, but the mobi conversion came back with this error:

*** TOC discontinuity: nodes are not sequential ***
node 028: '[32]' offset: 0x7D230 length: 0x9B1
node 029: '[BACK]' offset: 0x14D != 0x07DBE1
node data 028: {'titleOffset': 131, 'klass': 'chapter'}
node data 029: {'titleOffset': 136, 'klass': 'chapter'}
_generate_indexed_navpoints: Failed to generate index

and the bookmarks don't work. At least, the return bookmarks aren't working, the ones in the text do direct to the appropriate endnote.

Oh, BTW, this was from HTML to EPUB (worked) and HTML to MOBI (error). I'm using the <a name="stuff"> <a href ="#stuff"> formating

Thanks for any help anyone can give me!

Adam

EDIT: Found the underlying issue. Don't use <br/> (line breaks) use <p> paragraphs.
