Remove question mark icons
After editing images out of a book little blue question mark icons are left in it's place how do I remove them? Also can you link me to a tutorial of creating table of contents?

Okay I was going about removing the extra cover images the wrong way. I was deleting the image from the bottom section that's under images but what should be done is using the xHTML section at the top to remove the entire section that has the duplicate image in it and then save and that's it no more extra images!
