Calibre "forgetting" what books were sideloaded

I do want to add that I have done a few tests in order to troubleshoot:

- Set template to a very basic one: {series}/{title} - Vol.{series_index}
- Change title to include the series_index

They all failed and the book tried to match with Akira [2], I'm genuinely out of options. I might try to test with a fresh version of calibre in the hopes that that fixes it but otherwise, I'm just SOL.

Edit: a new fresh version of Calibre has made no differences, I'm just gonna wait for more advice/ideas from people.

What I would do at this point:

1. delete all the books from my ereader
2. rename all the titles in my calibre library, so that each of them is unique (e.g. "Akira 2", "Akira 3" and so on)
3. embed the metadata in the files, just in case
4. resend the books to my ereader

Do I understand correctly that you have already done this and Calibre still "forgot" the books on the device?
