No idea if it relates to your problem or not, but I discovered that putting a .nomedia file in the library directory to prevent photos/gallery apps finding cover images also prevented Calibre Sync from finding the cover images.

A 'hidden' .nomedia file in your library or its parent dirs could be your issue?
