I do prefer proper smallcaps. But the problem is then that I have to change the case of all the smallcaps in the ePub as in order to fake it, the publisher uses uppercase.

Did you read the comments in the XML code the OP posted? When he said smallcaps, it's not that he got smallcaps. It's that the fontsize was set to be about the size of smallcaps.
